CREDENTIALING DOCUMENTS.

In order to consider your request to join the Army Reserve Medical Corps as a Physician, you’ll need to provide us with certain documents. You can expedite the process by gathering together the following documents:

  • CV (Curriculum Vitae) of at least 2 pages, showing your contact information (including email address), Social Security Number, dates (month and year) of all schooling since the start of college, special research, past and current employment (list month and year) and any other pertinent information.
  • Proof of U.S. Citizenship/Permanent Resident Status (birth certificate, Naturalized Citizenship, or Green Card).
  • Licenses held in all states, including month, day and year of issuance.
  • Copies of your licenses, wallet-size preferred, showing the expiration date (please renew your license if there are less than 7 months of eligibility remaining).
  • Malpractice carriers for the past 7 years (list all, including names, addresses, telephone numbers and dates of coverage).
  • Malpractice suits, if any (list all, including description, dates, and outcomes).
  • Prior Military Service Records, if any. You'll need your DD-214 and DD-215 (if necessary) with the RE code showing at the bottom.
  • Medical School Transcripts, issued directly to your Army Reserve Physician Recruiter.
  • Three Letters of Reference, written by your peers (one from a department manager, if applicable), attesting to your skills and dedication as a Physician and affirming you will be a good candidate for consideration.
  • Motivation Statement, written by you, telling us why you want to become an Officer in the Army Reserve.
